Confused. What does this Histopathology report mean? What is this ?

Hello All,

In the last few months I have developed a few new moles, one of which was quite big pink flat with an irregular border, but all the moles seem to be pink/red inflamed and sensitive. I went to a dermatologist which took a biopsy on the big one on my back. No Melanoma was detected but it was identified as a Compound Naevus Dysplasie and recommended for complete removal. Here is the full report:

"This is a compound naevus with a lentiginous junctional component and scanty melanin. There are focal enlarged neats with bridging and proliferation of junctional melanocytes with focal atypia. The dermal component is benign with mild inflammation in the papillary dermis and increased vascularity. The appearances suggest dysplasia. No high grade cytological atypia or in situ malignancy seen. The lesion extends to the margins. A modest wider excision is advised."

Can anyone explain the above to me in Enlgish? The doctor could't. He just said it is nothing to worry about but would want to completely remove it and also get a second opinion on the Pathologist report? If there is nothing to worry about then why go through the trouble?

I have done a bit of reading on the internet and some sources suggest that a Dysplastic Nevu could turn into a MM at a later stage. But then what about all the other smaller moles that he didn't do a biopsy on that also seems inflamed? The dermatologist says not to worry about them as he doesn't want to cut me up too much, but I am worried. Should I stay out of the sun completely?

I also read some articles about Dysplastic nevi Syndrome. How do one get diagnosed with this? Do they have to discover lots of Dysplastic nevi?

Sorry for all the questions but I am very worried now. Any input would be much appreciated.

This is a compound naevus with a lentiginous junctional component and scanty melanin.

Tan or brown rounded spot on the shin was only a little melanin present.

There are focal enlarged neats with bridging and proliferation of junctional melanocytes with focal atypia.

I am not sure what “neats” means so I would be guessing if I tried to translate this sentence. Melanocytes are the skin cells that produce melanin so my guess is that there is some type of localized abnormality with them.

The dermal component is benign with mild inflammation in the papillary dermis and increased vascularity.

The dermis and papillary dermis are two of the skin layers. “Increased vascularity” means that there is some more blood vessels in the papillary dermis than normal.

The appearances suggest dysplasia.

Dysplasia means that it does not look right in terms of shape or size.

No high grade cytological atypia or in situ malignancy seen.

It does not have any cells that indicate that it is cancer.

The lesion extends to the margins. A modest wider excision is advised.

The sample did not remove all of the lesion so the pathologist is recommending that the doctor remove just a little more of the spot.

Of course, you need to talk with your medical team about this since I am not a doctor.

Thanks for the explanation Jim. I guess it means there is not much to worry about. But I will have another conversation with the dermatologist after he receives the second opinion.
