Insurance doesn't want to pay. Anyone else had this problem? What is this ?

My dad was diagnosed with prostate cancer at the end of last year. He is on his first dose of hormone therapy. His doctors at City of Hope say that the hormone therapy becomes ineffective after about 2 years of use. They have suggested my dad participate in a clinical trial for Helical radiation on top of the hormone therapy with hopes of full remission.
Now, his insurance had decided they don't have to pay for the radiation because it is beyond "normal" treatment.
If anyone else has faced this frustration I would love to hear the steps that you took to change their minds...
