I had a biopsy on a spot on my leg about 2 weeks ago the pathology came back and i dont really understand. Backgrond I'm a 34 year old male dark hair and fair to medium complexion. Dr thought when he removed the lesion that it was a thrombossed vein and that what it originally looked like. I have no family history of melanoma and only one cancer 82 year old grandfather passed away from colon cancer.
Here is the pathology report.
Diagnosis
Highly Atypical Compound Epitheloid melanocytic lesion.
Comment
This caase has been reviewd by multiple dermapahtologist. All who have reivew the case feel the lesion is bordreline with an inderterminate biological potential. The absence of diagnostic melanomain situ would favor an atypical lesion with the dermal growth pattern and cytologhy would favor invasive melanoma. .63mm. Reccomened full thisckness wide excision to ensure complete removal and adequate treatment.

Microscopic
The specimien is a punch biopsy of a compound melancytic pofiferation with a dominating intradermal component. A few small to medium sized nests of epitheliod melannocyts are irregulary distributed along the dermal/epidermal interfac and there is little upward movent of nests. Large iregualar shaped and varialbly sized nests composed of plump spindled melanocytes showing an tnedency toward confluence are seen in the upper portions of the dermis. a few small nests and individual cells are also scattered within the dermal component of the lesion. maturation is completed. Signifigant mitotic activity is not observed.

My question is do I have melanoma? Dr was very standoffish about my questions. He gave me book and told me to see a surgeon.

It sounds to me like they are unsure and are recommending a wide area excision just to err on the side of caution.
