Question on Bladder Cancer? What is this ?

My husband had the DaVinci Prostatectomy done Dec. 13th. The margins came back negative. His PSA level has returned to undetectible. But they found Adencarcinoma at the base of the bladder and at the Apex.
If it starts to spread will PSA levels go back up?

Right now it is just wait and see.

Yes, if your husband were to have a recurrence, PSA should start climbing. After surgery, PSA is a MUCH more useful screening tool.

If prostate cancer spreads elsewhere, it is still prostate cancer. Under a microscope, the cells would look like deformed prostate cells, and they produce PSA. So it wouldn't be bladder cancer per se, it would be prostate cancer in the bladder.

A study at Indiana University ( http://www3.interscience.wiley.com/cgi-bin/abstract/109594246/ABSTRACT?CRETRY=1&SRETRY=0 ) showed that bladder neck involvement is a risk factor for PSA recurrence, although it's not as bad as if there were seminal vesicles involved ( http://www.oncolink.com/resources/article.cfm?c=3&s=8&ss=23&id=756&month=11&year=2000 ) leading some researchers to question whether or not bladder neck involvement really puts the patient into a T4 stage or whether it's really more appropriate to be classified T3.

You will indeed want to monitor PSA closely, and be prepared to consult with a radiation oncologist and medical oncologist if you see a rise.

Don't take this as medical advice, of course. I'm just a patient with some research skills.
