Diagnosed prostate cancer Feb 15 2008. 12 core biopsy all gleason 7 3+4…. PSA 63 and nodal involvement 2 nodes. Started on casodex and then lupron 3 month, 4 months ago.
First PSA came back yesterday and was 1.0. Told uro I was hoping for .4 or less and he said….well 1.0 is pretty good. Asked him what is median time to PSA nadir and he says “you may have reached your nadir.
Does anyone know answer to how long to PSA nadir when on hormone only? |

Hi wocketman, I am very sorry about your cancer but happy that your treatments seem to be doing their job.
When it comes to PSA, I do not have an answer to your question. In general, it is not the actual number that matters but the overall trend. Since you PSA is going down, that is a very good sign. Just keep on doing what you are doing; it seems to be working

I don't know. I suggest checking in with online communities where there is a larger population of guys with advanced PCa. Prostate Cancer Infolink ( http://prostatecancerinfolink.net ) is one, and the Usenet group alt.support.cancer.prostate is another. The Usenet group can be subscribed to using a newsreader like Grabit or Newsbin, OR for quick and easy access, just go to the group via Google Groups at: http://groups.google.com/group/alt.support.cancer.prostate?lnk=
You should be aware that alt.support.cancer.prostate is not moderated in any way, and it's probably a good idea to read some of the message threads to get the lay of the land before posting.
My guess is that nadir in the ADT only setting is highly variable from patient to patient, and you might have multiple dips and rises. I know that some guys get Lupron only at first, PSA falls, then starts to rise, then they get Casodex on top of that, PSA falls again for awhile, etc...
You indicated that you're seeing a urologist. Most guys with advanced prostate cancer will probably tell you to see a medical oncologist instead, and one that specializes in prostate cancer if possible.
When my prostate cancer wasn't wiped out by surgery, I went to see a medical oncologist and radiation oncologist. Luckily for me, radiation seems to have worked. If my PSA begins to rise, though, I don't think I'll be going back to the urologist. I'll go see the medical oncologist.
Urologists do many things, operating on bladders, prostates, kidneys, etc...doing vasectomies, helping with ED problems, it goes on and on, and of course includes hormone therapy. Medical oncologists focus on things like hormone therapy and chemo--the treatments that will hopefully stall your cancer until it doesn't matter any more, or until new treatments are developed.

Searched pubmed.gov and came up with some answers. Seems I'm in the ballpark. Studies done in china, Japan and other places where the rules are fast and loose compared to here, but they were published in semi, to reputable journals.
Guess I gotta learn to forget about the next psa and the next, etc

Gotcha, actually have an appt with oncologist at Sloan Kettering in NY. Specializes in prostate and emerging therapies. Also have seen radio oncologist at Sloan on Long Island. One thing I will say for Sloan Kettering, arguably one of the best cancer hospitals in the country.....
They have the worlds largest database to draw on and have all kinds of nomograms, some of which are available online, as well as more charts and tables than you can shake a stick at.
They also have Howard Sher who was on the board for thumbs up or down for provenge.................and his thumb was down......guess he had his reasons
