Bowel cancer and spots on the liver

My uncle has been very sick for the last two weeks, although he has had bowel problems for the last 4 years. The doctors said that his bowel problems were related to back problems that he also has.

However, 2 weeks ago he was bleeding heavily when using the toilet and went to the doctor who said that he was anaemic and that he had internal bleeding. The doctor sent him to the hospital for an immediate colonoscopy due to the internal bleeding but they sent him home. He was sent into the hospital again last Monday and he was given an ultrasound on Friday and a colonoscopy on Saturday. The doctor confirmed that there was a tumour in his colon.

Today, he received a CAT Scan and the doctor said that there are spots on his liver also.

He is 58. The results were given orally on Saturday and today (Monday) and the test results are due to come back officially later this week. What can I expect? My father is in bits because he has accompanied this brother for numerous appointments for the last few years - yet this has gone undetected and reached the stage where a) It caused an obstruction and b) It has spread to his liver.

What will happen next? I have been furiously researching the topic since Saturday. If it has spread to the liver, does that mean that it is Grade IV cancer? The doctor said that they would surgically remove the tumour in his colon when my aunt spoke to him last Saturday - is this still going to be the typical procedure, or is a different procedure necessary now that it has spread?

I can see very poor statistics for this condition in my country (Ireland) once it is not caught at the early stages. Does "spots" on the liver mean that the cancer has spread? I think we are going to be completely in the dark until the written report comes back later in the week (if it comes back then...everything is so slow). What should I expect? What are his chances? Thanks in advance. I've found out a lot of information on this site already, but not sure if I fully understand the standard procedure in a case like this.

prayersforyou-hello prayers for you too. -Im sorry to hear your uncles diagnosis-with the tumor and bleeding it has gone to a later stage-your correct in saing untill the results come back one will not know. They may say it is an original colon cancer that has spread but it can also be two seperate cancers. More than likely it started in his colon. staging depends on how far it goes through the colon and if in fact it has spread to other sites. The doctor will discuss these facts when they get the results. They should then know if it should be shrunk first then operated orjust schedual removal of it-all depends on the location. If the doctor doesnt tell you take a notebook and pen write down questions such as what stage is it- is it removalble -what are treatment plans - is it the same cancer metasticized to the liver-what are the options of treatments-what are side effects.. then you go from there. You truly wont be able to research much untill you at least know that much

Hello sheila and thanks for your prompt reply.

It was good to get all the information down here. I suspected that the oral results given meant that it was at a later stage, but I hadn't looked into the topic enough initially to ask the right questions at the beginning. Thanks for the advice about bringing a notebook - I will do that as it is impossible to remember what is being said and remember quesitons too.

It's an awful shock, but I guess that once we have a bigger picture we can start to think of ways to treat it. It just feels like a long week waiting. Thanks for the advice

We have been told that he will have surgery in the next 2-3 weeks and that he will get chemotherapy after this. I hope that this is the best option. They want to do the surgery quickly as if the tumour moves (not sure if this is the correct terminology) it obstructs his colon, so they want to remove it quickly. It is 5cm in size (not sure if that is width/length) now.

He's having surgery tomorrow. Hopefully they will be able to remove the tumour. We were intially told that the tumour was in the early stages (in one hospital) but we were told that it could be there for 5/6 years in the hospital that he was transferred to. He had a colonoscopy 5 years ago, and the doctor said that they must have missed it then (different angle on the camera or something) which is quite depressing as he hasn't had a colonoscopy since as the doctors assumed his bowel problems related to a back problem that he also has... Anyway, hopefully it'll all go well tomorrow. Thanks again for the support

prayersforyou-I will be saying prayers for you and him-hope your getting some rest tonight-thats a long wait.
I really pray they can do something for him and that this will give you two more time together
It may not have been there when they did the test initially it may have been very tiny and undetectable and if it was growing all those years it may have been very slow as not to cause problems right away. Im sorry you both have to go through this wait and worry. (thats what I call it) because thats what Im always doing... waiting and worrying. But stay hopefull-He was strong enough to make it this far and is willing to do surgery and treatment -thats 1/3 of the battle! your support, his courage, your higher power(should you believe in one), his will to live-another 1/3 now the doctors skill and knowledge and innovative treatments and techniques----there you go weve got some hope please keep us posted on how the operation went and how you are too. take care

Thanks for your support and prayers Sheila. Surgery seems to have been a success although information was limited this evening and the patient was very sleepy! Do you have any tips on what foods/vitamins/supplements will help him to heal quickly? I've been looking online, but any extra advice would be very welcome. Delighted that things have moved so quickly this week and it's a huge relief to have the surgery over and done with. I'm really grateful that you have been so helpful

prayers for you-Im glad the surgery was successful. I wouldnt go into all kinds of vitamins/suppliments ,untill you see when meds he is going to be on. And once you do dont take it in your own hands to just try things. You will need to discuss every thing that you think may help with the doctor because altho things are really good for him, They may be harmfull with other drugs/chemicals he may have to take-you dont want to mess with that untill you find out all the facts. but hey at least there is hope now and thats nothing but a good thing. I hope he continues to heal and get his stregth back. untill then it may be awhile before he even feels like eating much at all. keep us posted ....get some questions ready for the doctor should you be able to see him.

I was talking to the nurse today and she said that his healing was normal and that they were giving him everything he needed to heal properly. Not sure if I'm completely satisfied with this as I know that he had bad healing before when he got his appendix out. They want to start chemotherapy as soon as possible (3-4 weeks if possible) so it's important that he heals quickly...when I talk to the doctor, should I ask if specific supplements would interfere with his medication?

Also, he has "bilobar" liver metastases. Does this mean that there are 2 tumours in his liver, or does it mean it is in two places? I've looked it up, but can't figure it out as I can't find a definition. Thanks again. He seems to be doing ok, but he's weak. I appreciate all your prayers and help

hoping he does well prayers for you. i assume bilobar means two lobes .

Thanks That would make sense alright unfortunately. I think that there are spots all over his liver, so it doesn't look too promising. He is recovering well from surgery though. Still weak and sore, but it seems to have been a success. Thanks for the support again.

It has spread to his lungs now too. Very bad news He had found it difficult to breathe since the operation so they did a scan during the week and we got the results today.

prayers for you-Im so sorry to hear this. what are the doctors saying ? are they able to buy him some more time? any treatment plan? depending on the severity of the spread treatments may prolong life awhile.

Well they want to start chemotherapy as soon as he is able for it - so I guess they're trying to buy him some time. The doctor spoke to my aunt today but he's has arranged a meeting for tomorrow to discuss the treatment plan. The doctor has given the choice of using a "TransPort Central Venous Catheter" as it is a new (and experimental??) treatment which could lessen the chances of infection. The other alternative seems to be a regular catheter. It's hard to know which one is the best choice.

He has been vomiting and has bad diarrhea at the moment. He doesn't want to eat much (although the only thing he can eat at the moment seems to be yogurt/rice pudding) and he's in a lot of pain as there's an infection and fluid on the wound. They did the scan on his lungs during the week as his blood pressure went crazy when they brought him for a walk. I think that as he was having trouble breathing, this must have put pressure on his heart.

He has lost a lot of weight and he's very unhappy in hospital. I'll know a bit more tomorrow. It's all very sad and as it has spread so much. Hopefully the doctor will have something positive to say tomorrow. At least they're still planning on doing chemotherapy - that must be something positive.

just remember if they come up with a treatment plan theres always some hope-they would put out time money or use those expensive treatments if they didnt think there was any chance. so thats a good thing. as far a choice of catheters? If hes having a central venous catheter-mom had one of those for awhile- she said it didnt really hurt getting it put in but it sure was better than them trying to constantly find a vein for the IV. It can stay in for quite awhile before having to be rplaced. not experimental and not all that new just newer than plain IVs I think the port may be newer than the central venous catheter. Its usually put in -in the operating room but thats only to use the monitors for guiding it and sanitary conditions. Doesnt take long.
