    Fully baked and came home with a friend

    Second and final round of HDR BT is complete. Had a minor issue and needed a foley. I will go back at 8 am to have it removed. The second round also had more discomfort after catheters were inserted, while with on radiation. Accepted pain meds but that was a mistake. I was goofy until bedtime. One good outcome of foley is I did not have to get up last night!

    Doc changed my meds from flowmax to Alfuzosin due to insomnia issues with Flowmax.

    I will see the doc in six weeks to ensure everything is on track and then at three months with first PSA. His plan is to then do PSA every six months until nadir is reached.

    I am really glad I chose this treatment, Dr. Biagioli, and Florida hospital. The medical staff are all amazing and they make treatment as comfortable as possible.


    Got to get this foley out so I can play golf today! Must have priorities. Denis
    65 YO healthy man
    PSA had been 4.1/2 for a couple of years,
    PSA 5/1/17 4.6,
    Multiparametric MRI, 5/15/17 showed lesion
    13 core needle biopsy 3 cores positive 3+3 and one positive in the lesion, may be overlap
    All cores less than 30%
    8/22/17 - second opinion pathology shows a small amount of (3+4) in one core, < 5%, ordered decipher to inform next steps
    9/27/17 -Decipher test shows intermediate risk so now exploring treatment options.
    2/6/18 - completed HDR BT
    5/3/18 Post HDR BT PSA 1.3
    9/18/18 PSA 1.2
    4/1/19 PSA 1.0 Testosterone 475

    PSA: Oct '09 = 1.91, Oct '11 = 2.79, Dec '11 = 2.98 (PSA, Free = 0.39ng/ml, % PSA Free = 13%)
    Referred to URO MD
    Jan '12: DRE = Positive: "Left induration"
    Jan '12: Biopsy = 6 of 12 Cores were Positive: 1 = Gleason 7 (3+4) and 5 = Gleason 6
    Referred to URO Surgeon
    March '12: Robotic RP: Left Positive Margins + EPEs. MD waited in surgery for preliminary Path Report then excised substantial left adjacent tissue(s) down to negative margins and placed 2 Ti clips for SR guidance, if needed in future.
    Pathology: Gleason (3+4) pT3a pNO pMX pRO / Prostate Size = 32 grams; Tumor = Bilateral; 20% / Perineural invasion: present
    3 month Post Op standard PSA = <0.1 ng/ml
    1st uPSA at 7 months Post Op = 0.018 ng/ml
    uPSA remains "stable" at 84 Months Post Op: Mean = 0.021 (20x uPSAs: Range 0.017 - 0.026) LabCorp: Ultrasensitive PSA: Roche ECLIA
    Continence = Very Good (≥ 99%)
    ED = present

    Feels good to be on the other side of treatment. Cath is out and no urinary burning. Doc told me to double the dose of AZO and it worked. Denis
